Families who received an acceptance letter from the DOE can download the two attached files to fill out the applications. Please sign the applications,and bring the completed /signed applications along with the requested documents to Al-Madinah School to finalize the registration. Families can also send us the complete and signed applications along with the documents via email [email protected].
Requested Documents:
- Birth Certificate
- 2 Proofs of address (Con Edison, National Grid, Lease , Tax Papers..etc)
-Physical Form and immunization record
-DOE Acceptance Letter
